Is it Normal Forgetfulness or Something More Serious?

As we grow older, many of us find ourselves becoming forgetful and worry it could be a sign of Alzheimer’s or another dementia disease. Researchers say it is true that the older we get we are more forgetful as the following reports: “At least half of those over age 65 say that they are more forgetful than they were when they were younger, experiencing ‘senior moments’ about things like where they put things or recalling somebody’s name.”

The question then becomes when what is the difference between “normal” age-related memory loss and Alzheimer’s disease. Studies find the brain shows warning signs of Alzheimer’s approximately fifteen years before someone experiences symptoms of mild cognitive impairment (MCI), and quite a while before the first real signs of dementia start.

Concerning signs of memory loss include not just forgetting an appointment or someone’s name, but repeatedly forgetting it. Another sign that something more serious is going on is if family members start to notice a loved one’s forgetfulness or confusion. Constantly repeating questions, forgetting the word for something or getting lost in a familiar neighborhood are other signs of a bigger problem. Judgement and planning may also be impaired, and tasks the person used to be good at like balancing a checkbook or following a recipe can become frustrating and difficult. 

To know for sure what is happening, you need to make an appointment with their doctor. They can do a complete medical work-up including reviewing the medications the person is taking, and checking for signs of depression and other environmental issues, which could account for symptoms. Seniors Who Are Lonely Are Higher Risk of Poor Health

According to the Centers for Disease Control & Prevention (CDC), statistics show seniors who report feelings of loneliness are at higher risk for several diseases including dementia and are more likely to experience premature death. The National Academies of Sciences, Engineering, and Medicine (NASEM) says “one-third of adults aged 45 and older feel lonely, and nearly one-fourth of adults aged 65 and older are considered to be socially isolated.” Social isolation is a lack of social connections and a result of a person living alone, having a spouse and/or other supportive friends or relatives die, or suffering from chronic illness. Other related problems include a lack of transportation, mobility impairment and untreated hearing loss, which all limit the senior’s opportunity to engage with others. 

AARP Research surveyed adults 45 and older regarding their feelings of loneliness. Of those respondents who rated their health as excellent, only 25 percent said they were lonely. Among participants who said their health was poor, 55 percent reported feeling lonely. The CDC also says studies find social isolation or loneliness is linked to a 29% increased risk of heart disease and a 32% increased risk of stroke as well as a 50% percent increased risk of dementia.

The Benefit of Eating Colorful Fruits & Vegetables

Research shows flavonoids, the chemicals that provide plant foods their bright colors, may help in the fight against forgetfulness and mild confusion many seniors experience. The evidence comes from two long-term continuing health studies from the late 1970s and early 1980s. Participants were periodically surveyed regarding their diet and state of their health including memory problems over a 20 year period. Data was extracted from over 100,000 men and women with an average age of 73.  

The study’s lead author, Dr. Deborah Blacker, a professor of epidemiology at the Harvard T.H. Chan School of Public Health, said they focused on participants’ consummation of two dozen common types of flavonoids. These included carrots (high in beta carotene), strawberries (containing flavone) and apples (containing anthocyanin). Results suggest those who answered “yes” to having memory problems also reported eating a lower amount of flavonoid-enriched foods.  

Researchers found a significant difference between the bottom one-fifth of participants who reported the lowest intake of flavonoids compared to the upper one-fifth who were 19 percent less likely to report forgetfulness or confusion. Additionally, the study reported “higher intakes of Brussel sprouts, strawberries, winter squash and raw spinach were most highly associated with better scores on the test of subjective cognitive decline.” Dr. Thomas M. Holland, a researcher at the Rush Institute for Healthy Aging who was not involved in the study, said “There’s some really good data here with 20 years of follow-up.”

Cataracts Are the Most Common Age-Related Eye Condition

The National Institute of Health (NIH) reports if you are lucky enough to live a long life, along the way, you will develop cataracts. While it may not get to the point of requiring surgery, they say everyone eventually has them. However, while cataracts are the most common age-related eye condition, they are also the leading cause of preventable blindness.

So how do you know you have cataracts or if it is time for surgery? Ophthalmologists explain the most accurate way to keep track of your eye health is to get regular eye exams; but there are some signs you can look out for on your own. One of the top changes you may notice is more difficulty driving at night. Cataracts make it much harder to see in the dark. Reading may also become difficult as you find yourself needing brighter and brighter light in order to focus on the page. 

Other symptoms can include seeing everything through a slightly tinted yellow or brown hue; or experiencing halos around light sources such as headlights or streetlights. Some cataracts can cause double vision, and between seeing halos and double vision make night driving especially dangerous. Experts say if you are experiencing any of these symptoms, please make an appointment to have your eyes checked.  

If surgery is recommended, remember every year an estimated 3.7 million cataract surgeries are performed in the United States. It is important to follow your doctor’s advice for a speedy recovery. Take Care When Starting an Exercise Routine

Doctors caution everyone, especially seniors, to go slow when starting or getting back into an exercise routine; and before you begin, to consult with your healthcare provider. After all you want to get back into shape, and causing injury or harm to yourself, is certain to delay or prevent this progress. 

Moreover, doctors reiterate that while individuals extremely out-of-shape will benefit the most from regular exercise, they are also more at risk for sudden heart problems resulting from such a workout. Dr. Martha Gulati, editor-in-chief of CardioSmart, the American College of Cardiology's patient education initiative, says it is important to "pace yourself, don't do too much too soon and make sure you give your body time to rest between workouts." 

One way to exercise safely is to pay attention to your heart rate. If you notice it is not coming down as your intensity decreases while resting or is beating irregularly, you need to stop. Dr. Gulati warns chest pains are never normal nor is nausea, vomiting or extreme sweating and if you experience symptoms, call 911. In addition, neither dizziness nor extreme shortness of breath is normal but rather a signal to stop and consult with your doctor. Cramps may be a sign of dehydration or low potassium levels and should not be ignored. Care should also be taken in individuals with atrial fibrillation, which is an irregular heartbeat, and seek emergency medical attention if such an event occurs.

Driving Safety for Seniors

Older drivers can still be good, safe drivers, but there are some things to consider both for your safety and the safety of others.  The Mayo Clinic recommends that seniors stay as active as possible in their day-to-day life which helps to maintain overall strength, flexibility, and range of motion.  Drivers need to be able to turn and look over their shoulder and move their head with ease.  They must be able to keep a firm grip on the steering wheel and legs need to be strong enough to apply pressure to the gas pedal or brake.  

A senior’s peripheral vision can decrease with age or from certain medications and other eye problems may cause drivers to have trouble seeing in the dark or experience blurred vision.  Hearing problems can interfere with detecting sounds of sirens from emergency vehicles or approaching cars.  Be vigilant about scheduling vision and hearing tests regularly as part of your wellness care and follow your doctor’s advice as to how often they should be done.  Many problems can be corrected if caught early enough.    

Remember that certain medications can cause drowsiness so make sure you know the side effects of your prescription drugs and do not drive if you are feeling sleepy.  It may be a good idea to limit trips in the car to daylight hours and when the weather is good.  Keep in mind that a person’s reaction time slows with age as do their motor skills.  You may need to re-evaluate your driving every few months, especially if you find yourself having incidents of accidents that almost happened.

Planning before you leave:

  • Plan to drive on streets you know.

  • Limit your trips to places that are easy to get to and close to home.

  • Take routes that let you avoid risky spots like ramps and left turns.

  • Add extra time for travel if driving conditions are bad.

  • Don't drive when you are stressed or tired.

While you are driving:

  • Always wear your seat belt.

  • Stay off the cell phone.

  • Avoid distractions such as listening to the radio or having conversations.

  • Leave a big space, at least two car lengths, between your car and the one in front of you. If you are driving at higher speeds or if the weather is bad, leave even more space between you and the next car.

  • Make sure there is enough space behind you. (Hint: if someone follows you too closely, slow down so that the person will pass you.)

  • Use your rear window defroster to keep the back window clear at all times.

  • Always keep your headlights on.

Tips for Better Communication with Your Doctor

Although it is first-hand, it is not always easy to describe to a doctor what is going on regarding medical problems. Describing various symptoms is quite all-together different from getting the results of actual lab tests or a doctor’s opinion after a physical exam.  

Relating symptoms clearly can help narrow down the problem, and medical experts share some advice on how to best do it. They start by telling patients to skip the medical jargon and just use their own words to describe what they are feeling. 

Dr. Lisa Ravindra, an assistant professor in the department of internal medicine and a primary care physician at Rush University Medical Center in Chicago, says, "Avoid using vague terms for timing like 'for a while' or stating that a symptom occurs 'once in a while' since these mean different things to different people." She explains it is helpful to be as precise as possible when talking about symptoms including the amount of time you have had them for such as three days, or on and off for a week. Doctors say it is helpful to rate your pain from one to ten. 

Other helpful things you can do include using a blood pressure monitor to chart your blood pressure, and if you feel feverish, take your temperature. Keep track of other important symptoms such as sore throat, earache, coughing, vomiting, diarrhea, etc. Be sure to mention unexpected weight loss, fatigue when doing things you typically do, or specific type and location of pain, i.e. a constant, sharp pain in the front of the right knee. If you have a chronic condition, doctors say it helps to keep a journal of symptoms to discuss with your doctor during your next visit.

They have many angles such as saying the individual has unpaid taxes or their Social Security or Medicare benefits are in danger of being cut off.  They then demand payment (in the case of unpaid taxes) or ask for personal identifying information, which is then used to commit fraud. These impersonators often “spoof” the actual phone numbers of the government agency, or call from the same area code for Washington, DC, tricking the victim into believing the call is legitimate.

The NCOA says there are ways to avoid getting scammed such as never giving out your personal information to anyone but a trusted family member, friend or your professional financial advisor.  In addition, never make big financial decisions without consulting with one of those trusted people.  Also, do not give out personal PIN numbers, or Medicare and other financial information over the phone unless you initiated the call or know the person.  

Other commonsense advice includes being wary of high-pressure sales persons, never signing a contract until you understand it entirely, and to be suspicious of unexpected announcements of winning money, trips, or other prizes.  Experts say it is most likely a hoax.  Always take your time when making financial decisions and do not give blanket approval to a vendor for medical equipment and other purchases.

Dr. Mark Albers, a neurologist at the McCance Center for Brain Health at Harvard - affiliated with the Massachusetts General Hospital, says it is not all that uncommon for seniors to experience symptoms of brain fog when taking certain medications for the first time. He explains medications can affect memory more in aging adults as “older people tend to metabolize drugs more slowly, making them more sensitive to medications and more vulnerable to side effects.” 

Other reasons medications cause more problems in seniors are they often take larger doses of a drug and more frequently as well as take several drugs to treat one health condition. Moreover, older people may have a weakened blood-brain barrier. This barrier works to block toxins and other harmful substances including drugs from entering the brain, which may account for some memory problems. 

Sleep medications are one of the common medications that can be problematic. Over-the-counter sleeping aids can contain anticholinergic drugs, which are known to impair cognitive function, and some prescription sleep drugs diminish activity in parts of the brain involved in transferring short-term memories to long-term memory. Pain medications can additionally result in memory problems. The good news is once patients stop taking sleep or pain meds, modify the dose or replace the drug with a better option, many of the problems go away.
